A damning report says 520 mothers and babies suffered “potentially avoidable” harm or death at hospitals in Nottingham.

“Cruel", "toxic, and “dismissive” are just three of the words used to describe the culture at the Nottingham University Hospitals Trust.
It blamed "deeply embedded systematic failures", highlighting a pattern of families not being listened to or believed and where bullying and cruelty to patients was common.
Understaffing was routine, senior management repeatedly ignored warnings and failed to act, and lessons were not learned, even when the most catastrophic failures had occurred.
