One Steering Failure Broke the Whole Connection
In 2004, the tug Salvor was pushing the tank barge KTC-115 when a steering failure created a violent shock load. The heavy wires and chains holding the tug in the notch began to part.
With nothing left securing the connection, the barge grounded and spilled about 60 tonnes of cargo. The notch did its job, the rigging did not.
